A consultation appointment will be made.
Initial consultation is free of charge. |

A sampling appointment is made. Appropriate
identification will be required for the completion of the approved paperwork prior to sampling. |

Sampling will be carried out by one of our
company registered nurses. A cheek swab will be taken from all individuals required to be tested.
Clients can get samples taken by their own GPs but this will be charged at the discretion of the GP. |

Samples will be sent to an ISO 17025
accredited laboratory for testing. |
| 6. |
Results will be obtained within 10 working
days from the date of obtaining the last sample at the testing laboratory. |

Another appointment is made between the clients
and Polygenyn. At this consultation the client will be presented with a report of the results
and a detailed explanation of what the results actually mean. Further referral to a qualified counsellor would be suggested
if required. |

A valid form of identification will be required. This may take the form of passport or drivers licence. |

In private cases individuals can identify each other i.e. mother can identify children however for our records photographs will have to be taken. |

GPs can also identify individuals if they are known to the GP. |

Documentation is to be completed and signed by the client and the sampler at the time of sampling |

The case will then be processed by Polygenyn or sent by the clients GP to Polygenyn for processing |
